Chicago, IL- The Illinois Department of Public Health recently released their quarterly report of nursing home violations in Chicago. This report can be a valuable tool for families who are considering a nursing home for their loved one or suspect they are being neglected.

 Nursing Home Violations for 2017 First Quarter

The IDPH report lists 28 nursing homes in Illinois that were cited for serious violations in January, February, and March of this year. The violations are the result of residents in those facilities suffering a harmful or fatal injury. Some of the incidents involved a resident choking to death, one resident fell and suffered a brain injury, and another was verbally and physically abused by two staff members.

Nursing homes face violations when they fail to give residents proper care or fail to keep them from being harmed.

The report lists Illinois nursing homes with “A” violations and “AA” violations for the first quarter of 2017. “A” violations include conditions that make it likely a resident could suffer harm or hasten their death. “AA” violations are incidents that directly result in a resident’s death.

Ambassador Nursing & Rehab- 4900 North Bernard, Chicago

Alden Lakeland Rehab & Health- 820 West Lawrence, Chicago

Both facilities have requested a hearing to contest the violations which can result in result in fines up to $25,000.
